Once completed, print or save your certificate.

1- Print this form and visit AccuTest Drug Testing & LiveScan Services at 201 S Guild Ave, Suite 101, Lodi CA (next to Tin Roof BBQ)
2- Print this form. This documents explains that Lodi Christian School will be checking your background every six months to make sure you still meet the criteria for volunteering with students. Email or return completed form to the office.
Once completed, LiveScan will notify us.
The cost can be reimbursed by the school.
Driving for a Field Trip
To drive for a field trip we require this form to be completed along with a copy of

Insurance ID card showing effective dates of coverage

Insurance Declaration page showing limits of coverage
